Hello Mr. Brown,

 

Attached to this letter is TVO’s standard release form that we ask guests of The Agenda with Steve Paikin to sign in regards to their appearance on the program or a web exclusive.

 

Since TVO is an educational broadcaster, we often re-use the interviews we conduct on The Agenda for use in schools and on our educational websites. To that end, your release confirms that we are able to broadcast excerpts of The Agenda that includes your appearance on the program.

 

We ask that you please complete and sign the form. Please also have a witness sign the form. We then ask that you fax the form back to TVO at (416) 484-4519, ATTN: Nikki Ashworth. Alternatively, if you have a scanner, you may also choose to scan the signed form in PDF file-format and you may then send it back to nashworth@tvo.org. Please do not feel you have to rush to get the release back to us before your taping.
