I think it's possible only if there is a connection between the above and below ground. However, it's easy to accidentally tunnel into the underground. Otherwise the above ground is as tame as usual.

One of the most exciting parts is that they put a lot more of the variables into the raws so the game is more mod-able. We should start seeing some spectacular new versions soon.

You need to focus your defenses downward, and don't dig too deep until you're ready to defend yourself. In my first fort, a poisonous Great Coral Python forgotten beast killed my entire fortress of 50ish dwarves, even though they already had a well trained squad with steel armor and weapons.
